How do I submit my lab results?

If you have your own labs, you can upload them directly to your account here: https://app.trymeasured.com/lab-results

Please make sure that your lab work is is from the last 12 months and has the results for: 

• Creatinine
• eGFR
• Hemoglobin A1c
• Lipids levels

If you have completed the Measured Lab Kit, please follow the return instruction located in your kit. If you lost the instructions or have any questions, please chat with our care team, we will be happy to support you. 

Please note that your lab results are required for you to proceed with Measured. If you fail to complete and/or submit your labs, you will not be able to continue with your Measured treatment plan or request a refill for your medication.
