What information do we need for a prior authorization?

In order to have the best possible outcome for the medication to meet the insurance company’s criteria, we need an accurate medical history that includes allergy history, any diagnosed medical conditions, any tried and failed medications for weight loss and the dates of trial and failure. If the dates are not available, you can provide the trial length for each medication. Ex. 3 months
